29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) On or about May 18, 2012, at 7411A Remcon Circle in El Paso, TX, the scaffold two employees were operating from was not fully planked. This condition exposed the employees to a fall hazard of approximately 13 feet to the ground below.

29 CFR 1926.452(c)(2): Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would secure vertical members together laterally, and the cross braces were not of such length as would automatically square: On or about May 18, 2012, at 7411A Remcon Circle, El Paso, TX, two employees were working on the scaffolds which were assembled without any cross bracing. This condition was exposing the employees a fall hazard of approximately 13 feet to the ground below.

This condition exposed the employees to a fall hazard.
