29 CFR 1910.147(c)(1):     Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    a.) Containercraft, Inc. located at 144 Plymouth St. Lexington, OH; On or about May 18, 2012, the employer did not assure an energy control program consisting of employee training, periodic inspections, and machine specific procedures, was established.  Employees were exposed to caught by and amputation hazards while performing setup and maintenance activities on equipment including, but not limited to; McKinley Flexofolder, General Taper, Curioni Press, Chicago and New York die cutter, McKinley rotary die cut, and the S&S Eccentric Slotter.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a.) Containercraft, Inc. located at 144 Plymouth St. Lexington, OH; On or about May 18, 2011, the employer did not assure employees were protected from the rotating wheel on the Chicago and New York Flat Die Cut machine. Employees were exposed to caught by hazards while walking back and forth to feed corrugated material and help catch the finished boxes.    b.) Containercraft, Inc. located at 144 Plymouth St. Lexington, OH; On or about May 18, 2011, the employer did not assure employees were protected from the rotating wheel on the S&S Eccentric Slotter. Employees were exposed to caught by hazards while accessing the small table attached on the right side of the press.    c.) Containercraft, Inc. located at 144 Plymouth St. Lexington, OH; On or about May 18, 2011, the employer did not assure employees were protected from caught by hazards on the Conveyor Metal Works, Inc. portable conveyor. Employees were exposed to caught by hazards while using the conveyor to pick up and stack parts at the end of the McKinley Rotary Die Cut Machine.

Containercraft, Inc. located at 144 Plymouth St. Lexington, OH; On or about May 18, 2012, the employer did not certify employees were trained and evaluated to operate a forklift. Employees were exposed to struck by hazards while moving corrugated materials to and from machines and on and off semi trailers.
