Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following hazards:     (a)	Great Homes, Ltd at 3032 I-70 Business Loop, Grand Junction, CO 81504: On and before 4/24/12, the employer did not ensure that employees were properly trained to use ladders:    1)	Employee overloaded ladders beyond their maximum intended load or beyond their manufacturers rated capacity.    Conditions exposed employees to a fall hazard and struck by hazards from falling debris.     Abatement Note: Among other methods, American National Standards Institute (ANSI) ASC A14.2-2007 provides that one feasible and acceptable abatement method includes providing training, care and requiring its use of portable metal ladders of various types and styles.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a)	Great Homes, Ltd at 3032 I-70 Business Loop, Grand Junction, CO 81504: On and before 4/24/12, the employer did not ensure that protective equipment was used in the following instances:    1)	Two employees using chainsaws on the ground without the use cut-resistant clothing.    Conditions exposed employees to the hazards of projectiles to the eyes, to hearing loss, cuts and amputations, and struck by hazards from falling debris.     Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).    				OR IN THE ALTERNATIVE    Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following hazards:     (a)	Great Homes, Ltd at 3032 I-70 Business Loop, Grand Junction, CO 81504: On and before 4/24/12, the employer did not ensure that protective equipment was used in the following instances:    1)	Two employees were not wearing safety glasses when working around chain saws;  2)	Three employees were not wearing hearing protection when working around chain saws;  3)	Three employees were not wearing hard hats; including employees working on the ground under an employee using a chain saw in a tree.   4)	Two employees using chainsaws on the ground without the use cut-resistant chaps.    Conditions exposed employees to the hazards of projectiles to the eyes, to hearing loss, cuts and amputations, and struck by hazards from falling debris.     Abatement Note: Among other methods, American National Standards Institute (ANSI) Z133.1  2000 Section 4.0 provides that one feasible and acceptable abatement method includes providing personal protective equipment and requiring its use.

Employees performing tree trimming tasks did not wear safety glasses when working around a chain saw.  This condition exposed employees to eye injuries. (a)	Great Homes, Ltd at 3032 I-70 Business Loop, Grand Junction, CO 81504: On and before 4/24/12, the employer shall ensure that each affected employee wears a protective helmet when working in areas where there is a potential for injury to the head from falling objects. Three employees were not wearing hard hats; including employees working on the ground under an employee using a chain saw in a tree. This condition exposed employees to struck by hazards from falling debris.     Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).
