29 CFR 1918.24(i)(1): The employer did not maintain portable ladders in safe condition. a) Tuna Fishing boat Koorale, bow, store fish well #2 and #3: On or about 07 APR 12, employees were observed using defective ladder during unloading the vessel. One of the ladders had been repaired with a piece of aluminum stock and then duct-taped while another exhibited sharp torn edges where the ladder had broken into. Employees could have cut themselves while going up and down the ladders or fallen.
