29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface to which the ladder was used to gain access: Employees were exposed to a fall hazard of approximately 11 feet to the ground below when using a ladder to access the roof and the side rails did not extend at least 3 feet above the landing surface.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure: Employees were exposed to a fall hazard of up to approximately 11 feet to the ground below while removing and installing roofing material and no fall protection was being used. STEVE TIBBS D.B.A. TIBBS CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD: 29 CFR 1926.501(B)(13)WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312894546, CITATION NUMBER 1, ITEM NUMBER 2A AND WAS AFFIRMED AS A FINAL ORDER ON 09/28/2010 WITH RESPECT TO A WORKPLACE LOCATED AT 1902 ST.
