29 CFR 1926.55(a): Employee(s) were exposed to material(s) at concentrations above those specified in the Threshold Limit Values of Airborne Contaminants for 1970 of the American Conference of Governmental Industrial Hygienists: a. On or about May 21, 2012, at the jobsite located at 288 Orland Square Drive, Orland Park, IL, an employee was exposed to respirable silica while operating a Prep Master floor grinder. The employee was exposed to respirable dust containing 6.9 % crystalline silica (quartz) at an 8-hour time-weighted average level of 25.47 millions of particles per cubic foot (mppcf), approximately 1.2 times the permissible limit (PEL) of 20.99 mppcf. This limit was established to prevent silicosis. b. On or about May 21, 2012, at the jobsite located at 288 Orland Square Drive, Orland Park, IL, an employee was exposed to respirable silica while sweeping, shoveling and picking up broken tile. The employee was exposed to respirable dust containing 4.76 % crystalline silica (quartz) at an 8-hour time-weighted average level of 31.81 millions of particles per cubic foot (mppcf), approximately 1.2 times the permissible limit (PEL) of 25.60 mppcf. This limit was established to prevent silicosis. c. On or about May 21, 2012, at the jobsite located at 288 Orland Square Drive, Orland Park, IL, an employee was exposed to respirable silica while operating an Innovatech Terminator propane powered flooring removal machine. The employee was exposed to respirable dust containing 3.76 % crystalline silica (quartz) at an 8-hour time-weighted average level of 29.29 millions of particles per cubic foot (mppcf), approximately 1.03 times the permissible limit (PEL) of 28.55 mppcf. This limit was established to prevent silicosis.

The employee was exposed to respirable dust containing 6.9 % crystalline silica (quartz) at an 8-hour time-weighted average level of 25.47 millions of particles per cubic foot (mppcf), approximately 1.2 times the permissible limit (PEL) of 20.99 mppcf. This limit was established to prevent silicosis. 1. General engineering methods of control applicable in these circumstances include, but are not limited to, the following: a. Use more water during the concrete floor grinding and floor tile removal. b. Use multiple negative air machines in the containment. c. Ensure and verify that the engineering control on the concrete floor grinder is operating in accordance with manufacturing specifications. 2. Administrative controls applicable in these circumstances include, but are not limited to, the following: Rotation of employees during floor tile removal operations and floor grinding operations to reduce employee exposure to below the PEL. ABATEMENT NORMALLY WILL BE MULTISTEP AS FOLLOWS: STEP 1: EFFECTIVE RESPIRATORY PROTECTION IN ACCORDANCE WITH 29 CFR 1910.134 SHALL BE PROVIDED AND USED BY EXPOSED EMPLOYEES AS INTERIM PROTECTIVE MEASURE UNTIL FEASIBLE ENGINEERING AND/OR ADMINISTRATIVE CONTROLS CAN BE IMPLEMENTED, OR WHENEVER SUCH CONTROLS FAIL TO REDUCE EMPLOYEE EXPOSURE TO WITHIN PERMISSIBLE EXPOSURE LIMITS. ABATEMENT PERIOD: 1 DAY 07/25/2012 STEP 2: SUBMIT TO THE AREA DIRECTOR A WRITTEN, DETAILED PLAN OF ABATEMENT OUTLINING A SCHEDULE FOR THE IMPLENTATION OF ENGINEERING AND/OR ADMINISTRATIVE MEASURES TO CONTROL EMPLOYEE EXPOSURE TO HAZARDOUS SUBSTANCES AS REFERENCED IN THIS CITATION. THIS PLAN SHALL INCLUDE, AT A MINIMUM, TARGET DATES FOR THE FOLLOWING ACTIONS WHICH MUST BE CONSISTENT WITH THE ABATEMENT DATES REQUIRED BY THIS CITATION: 1. EVALUATION OF ENGINEERING/ADMINISTRATIVE CONTROL OPTIONS; 2. SELECTION OF OPTIMUM CONTROL METHODS AND COMPLETION OF DESIGN; 3. PROCUREMENT, INSTALLATION AND OPERATON OF SELECTED CONTROL MEASURES; AND 4. TESTING AND ACCEPTANCE OR MODIFICATION/REDESIGN OF CONTROLS ABATEMENT PERIOD: 30 DAYS 09/05/2012 NOTE: ALL PROPOSED CONTROL MEASURES SHALL BE APPROVED FOR EACH PARTICULAR USE BY A COMPETENT INDUSTRIAL HYGIENIST OR OTHER TECHNICAL QUALIFIED PERSON. Employees were wearing disposable N95 filtering facepieces. An employee was exposed to respirable silica while operating a Prep Master floor grinder. The employee was exposed to respirable dust containing 6.9 % crystalline silica (quartz) at an 8-hour time-weighted average level of 25.47 millions of particles per cubic foot (mppcf), approximately 1.2 times the permissible limit (PEL) of 20.99 mppcf.
