29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) On or about 5/15/12, on construction jobsite at First Niagara Bank, 2421 Main St., Buffalo, NY: An employee was exposed to potential fall of approx. 28 ft. while screwing plywood down on scaffolding that was erected in the front of the building. The employee was doing this task while in the elevated "basket" of a "JLG Model 400 S" aerial lift. The employee was wearing a full body harness with a lanyard, but the lanyard was not attached to the basket. NO ABATEMENT CERTIFICATION REQUIRED
