29 CFR 1926.1053(b)(7): Ladders shall not be used on slippery surfaces unless secured or provided with slip-resistant feet to prevent accidental displacement. Slip-resistant feet shall not be used as a substitute for care in placing, lashing, or holding a ladder that is used upon slippery surfaces including, but not limited to, flat metal or concrete surfaces that are constructed so they cannot be prevented from becoming slippery. From January 25, 2012 until March 27, 2012 worker(s) were engaged in commercial construction activities renovating the interior of an existing structure. The project included demolition work and the construction of a upper level mezzanine area. Worker(s) were exposed to potential fall hazards in that the extension ladder used to access and egress the mezzanine area was not properly secured.

From january 25, 2012 until March 27, 2012 worker(s) were engaged in commercial construction renovating the interior of an existing structure.The renovation project included demolition work and the construction of an upper level mezzanine area. Worker(s) were exposed to potential fall hazards in that the extension ladder utilzed to access and egress the mezzanine area was not properly inspected to identify possible defective parts.

From January 25, 2012 until March 27, 2012 worker(s) were exposed to potential fall hazards in that the extension ladder utilzed to access and egress the mezzanine area was not properly inspected, tagged, or taken out of service.
