29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder: a. On or about May 18, 2012, five employees used a 28 foot Werner extension ladder to access a roof that did not extend 3 feet over the eave, exposing themselves to an 8.2 feet fall hazard. Abatement Certification and Documentation are not required

AAA Roofmasters Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 315478628, citation 01, item number 001, issued on August 5, 2011, with respect to a work site at 1700 Mindanao Drive, Jacksonville, Florida 32246. The final order date was August 19, 2011. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a. On or about May 18, 2012, on a 4:12 pitch roof, five employees replacing a roof were not protected from an 8.2 feet fall hazard by use of a guardrail system, safety net system, personal fall arrest system, or any alternative fall protection system. Abatement Certification and Documentation are not required
