No personal fall arrest system or other means of fall protection, such as guardrails, was provided by the employer. Condition(s) were noted on or about 5/17/2012.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: A metal extension ladder leaned against the east edge of the flat roof provided access to / from the 11 ft high lower flat roof by employees. The side rails did not extend at least 3 ft above the landing, nor was alternative fixed hand-hold provided for steadying workers getting on / off top of ladder. Condition(s) were noted on or about 5/17/2012.

A metal extension ladder leaned against the east edge of the flat roof of new building under construction at 632 Amboy Ave in Perth Amboy provided access to / from the 11? ft high lower flat roof by carpentry employee(s) for framing and/or sheeting work. The feet of the ladder rested unevenly upon an area of soil & concrete rubble; causing the ladder to be out-of-plumb with vertical when viewed from the front and contributing to employee fall hazard exposure(s). Condition(s) were noted on or about 5/17/2012.
