29 CFR 1926.451(c)(2)(iv):  Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use.    a)  Front of Business Establishment:  Employer used a Daewoo LP Type forklift to elevate employees on a temporary work platform without this equipment being specifically designed by the manufacturer for such use, on or about 3/29/2012.

29 CFR 1926.451(c)(2)(v):  The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied.    a)  Front of Business Establishment:  Employer used a Daewoo LP Type forklift to elevate employees on a temporary work platform without ensuring that the entire platform was attached to the fork.  With employees elevated approximately 14' above ground surface, forklift was moved horizontally, on or about 3/29/2012.

29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    a)  Front of Business Establishment:  Employees, working on elevated scaffold platform approximately 14 above a lower point of access, were without means to access this point.  Scaffold platform had been elevated by a Daewoo forklift which soon after had been left unattended, on or about 3/29/2012.
