29 CFR 1910.23(d)(1)(iii): Stairway(s) less than 44 inches wide having both sides open, did not have one stair railing on each side:  (a)  On May 25, 2012, at the facility jobsite located at 1106 Guionette, Kansas City, MO., The stairway used for entry/exit next to the loading dock, did not have a complete stair rail system on each open side of the stairs. Employees were exposed to fall hazards injuries in the event of a fall from the stairs.         Abatement certification is required for this item  use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)  On May 25, 2012, the employer did not ensure that Nardini TT-1030-E Metal Lathe located in the maintenance room was equipped with a guard on the rotating chuck head. Employees were exposed to serious struck by injuries from the unguarded rotating chuck head.       Abatement certification is required for this item  use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a)  On May 25, 2012, the employer did not ensure that electrical equipment located in the maintenance room was used in accordance with listing and labeling instructions, in that two Relocatable Power Taps had been connected together in a daisy chain manner. Employees were exposed serious injuries in the event of a power overload to the electrical equipment and or the branch outlet which could create a fire or electrical shock.      Abatement certification for this violation is not required.

29 CFR 1910.305(a)(2)(ix): Lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard:  (a)  On May 25, 2012, the employer did not ensure that the over head florescent lixtures located in the Maintenance and Production room areas, were protected from accidental contact or breakage. Employees working under and around the light fixtures were exposed to serious injuries to eyes and and or cuts if the fixtures were accidentally broken.
