29 CFR 1910.23(d)(1)(iii): Stairway(s) less than 44 inches wide having both sides open, did not have one stair railing on each side: (a) On May 25, 2012, at the facility jobsite located at 1106 Guionette, Kansas City, MO., The stairway used for entry/exit next to the loading dock, did not have a complete stair rail system on each open side of the stairs. Employees were exposed to fall hazards injuries in the event of a fall from the stairs. Abatement certification is required for this item use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

Employees were exposed to serious struck by injuries from the unguarded rotating chuck head. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) On May 25, 2012, the employer did not ensure that electrical equipment located in the maintenance room was used in accordance with listing and labeling instructions, in that two Relocatable Power Taps had been connected together in a daisy chain manner. Employees were exposed serious injuries in the event of a power overload to the electrical equipment and or the branch outlet which could create a fire or electrical shock. Abatement certification for this violation is not required.
