29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms: a) Allied Exhaust Systems Inc. at 4876 Leaf Court, Denver, CO 80216: On or before May 7, 2012, the employer did not ensure that the stairs in the warehouse, used to access the mezzanine, included a midrail. The steps consisted of 16 steps 36 inches wide, and were open on one side with a 33 inch high metal handrail. Condition exposed employees to fall hazards of up to 10'6". Abatement note: 1910.23(e)(1) provides that a standard railing shall consist of a top rail, imtermediate rail, and posts.

29 CFR 1910.26(c)(2)(vii): Ladders having defects were not marked and taken out of service until repaired by either the maintenance department or the manufacturer: a) Allied Exhaust Systems Inc. at 4876 leaf Court, Denver, CO 80216: On or before May 7, 2012, the employer did not ensure that ladders having defects were marked and taken out of service in the following instances: 1) An older platform ladder, # 46-00579, was missing a vertical support by the first step, and its brakes were not working; 2) A newer platform ladder, ID not available, had a vertical support bent 5 degrees; and 3) A 3 foot Easy Reach ladder, model AS-3, had a cracked top platform. Condition exposed employees to fall hazards of up to 66 inches.
