29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. A) On or about 05/10/2012 excessive amounts of corn ear leaf, silks, husks and stems were observed on the work floor of the corn cutting room. This created a tripping hazard to the employees.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1): a) On or about 05/10/2012, two flexible electrical extension cord feeding the Win-Holt film wrapper machines (Model WHSS-1), were observed pinched under several pallets. The weight of pallets was damaging the insulation capabilities of the extension cords.
