29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: The employer does not protect employees from head injuries sustained by objects falling or flying off the roof. This violation was observed on or about May 16, 2012, in the parking lot of a commercial building where employees were exposed to falling objects when propane cylinders and torches were lowered while the employees were not wearing hard hats. COBBLESTONE ROOFING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.100(a), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER, 314426487, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON 10/29/10 WITH RESPECT TO A WORKPLACE LOCATED AT 116511 HEDGECROFT, HOUSTON, TX 77060. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are provided protective helmets to guard against head injuries from falling or flying objects.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. The employer does not protect employees from falling when working 6 feet or move above lower levels. This violation was observed on or about May 16, 2012, on the flat roof of a commercial building where employees were exposed to a 23 foot fall hazard when performing roofing work without a fall protection system. COBBLESTONE ROOFING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b) (10) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314426487, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIREMED AS A FINAL ORDER ON 10/29/10, WITH RESPECT TO A WORKPLACE LOCATED AT 116511 hEDGECROFT, HOUSTON, TX 77060.
