29 CFR 1903.19(d)(1):     The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.    The employer failed to provide documentation that the employees exposed to electrical shock while delivering construction materials had received training on safety-related work proactices for preventing exposure to energized power lines.

29 CFR 1926.600(a)(6)(i):  For lines rated 50 kV or below, minimum clearance between the lines and any part of the crane or load was not maintained at a distance of at least 10 feet:    On or about April 23, 2012, 10 feet (305 cm) of clearance was not maintained between 7620 volt overhead power lines and the boom of the 1999 Sterling crane truck (VIN 2FZXKWEB7XAB34028) while this truck was being used to lift construction materials to a roof.
