29 CFR 1926.501(b)(10): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) Jobsite, 2295 Prebble Road, Pittsburgh, PA: On or about May 15, 2012, during roofing work on a low sloped roof, the employer did not ensure that employees used appropriate fall protection systems/devices where exposed to falls of approximately 25 to 30 feet in that the employees used the parapets walls for fall protection, which were determined to be approximately 37" in height.

29 CFR 1926.502(i)(2): All other covers were not capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time: a) Jobsite, 2295 Prebble Road, Pittsburgh, PA: On or about May 15, 2012, during roofing work, the employer did not ensure that roof openings had covers of sufficient strength to prevent falls in that a series of skylight domes were in place over skylight roof openings, but the domes were not rated for impact or fall prevention.
