29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through written certification that identified the workplace evaluated; the person certifying that the evaluation had been performed; the date(s) of the hazard assessment; and, identified the document as a certification of the hazard assessment: a. Maintenance activities - a written certification was not developed for employees using personal protective equipment such as, but not limited, safety glasses and chemical protective gloves. Violation observed on or about 05/15/12. Beacon Woods Golf Course, Inc. was previously cited for violations of the Occupational Safety and Health Standard or it's equivalent standard 1910.132(d)(2) which was contained in OSHA Inspection Number 311335863, Citation Number 1 Item Number 1b, issued on 01/10/2008, and became final order on 02/06/2008 with respect to a workplace located on 12507 Clock Tower Parkway, Hudson, FL 34667.
