29 CFR 1910.23(c)(1)  Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:  1)  Molding area, machines including but not limited to Molding machine #3 used daily by employees, was missing a standard guard rail on the platform where the access ladder ascends to the platform.  (All fall protection guarding shall have a top rail and intermediate rail.)

29 CFR 1910.178(l)(2)(ii) The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace (An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years):  1)  At the facility, for forklift operators who had never received the practical training and operator's performance evaluation.
