A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. a. On or about May 21, 2012, at the above addressed jobsite, employees conducting sewer installation in a trench six (6') feet in depth did not have a safe means of egress.

29 CFR 1926.651(i)(3): Sidewalks, pavements and appurtenant structure shall not be undermined unless a support system or another method of protection is provided to protect employees from the possible collapse of such structures. a. On or about May 21, 2012, at the above addressed jobsite, employees conducting sewer installation operations undermined the pavement by approximately five (5) feet, thereby exposing employees to the hazards associated with a street collapse.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: a. On or about May 21, 2012, at the above addressed jobsite, employees were conducting sewer operations in a trench measured at seven (6) feet in depth. The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards.
