OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:    (a)  On or about 5/16/2012, at the duct cutting and forming department, Duct Design Corporation, Hialeah, FL.  Employees were exposed to a caught by hazard while working with a hydraulic press brake (Model 10) without point of operation guarding and/or alternate measures which provide effective protection.    Note:  Certification of abatement must be submitted for this citation item.

(a)  At the duct cutting and forming department, employees were exposed to an electrical hazard while working in close proximity to an electrical fan (Emerson, Model K55HXLW0-3929) that lacked a ground prong on its attachment plug.      (b)  At the duct assembly department, employees were exposed to an electrical hazard while working in close proximity to an electrical fan (Model HVP-30E) that lacked a ground prong on its attachment plug.     (c)  At the duct assembly department, an employee was exposed to an electrical hazard while working with a drill press (Northern Industrial, Model 17209) that lacked a ground prong on its attachment plug.
