(b) On or aobut 5/22/12 - Clear Color line, the discharge end chute of the conveyor was inadequately guarded to protect employees from falling into the conveyed system.    (c)  On or about 6/6/12 - Multisort area, when the door was opened for cleaning of the machine, an opening was inadequately guarded to protect the employee from falling into the machine.
