Employees using the press to manufacture paving breaking chisels were exposed to a caught-by hazard. b) Heavy Forging Area, No. 2 Press Trim, 1466 Delberts Drive, Monongahela, PA 15063: On or about May 11, 2012, the No. 2 Press Trim full revolution mechanical power press was not incorporated with a single-stroke mechanism. Employees using the press to manufacture paving breaking chisels were exposed to a caught-by hazard. c) Heavy Forging Area, Nazel Trim Press, 1466 Delberts Drive, Monongahela, PA 15063: On or about May 11, 2012, the Nazel Trim full revolution mechanical power press was not incorporated with a single-stroke mechanism. The portion of flywheel facing the press was not guarded. The flywheel was approximately fifty four inches from the floor. Employees using the press to manufacture paving breaking chisels were exposed to caught-by hazards. b) Heavy Forging Area, No. 2 Press Trim, 1466 Delberts Drive, Monongahela, PA 15063: On or about May 11, 2012, the flywheel on the No. 2 Press Trim was not fully guarded. The portion of flywheel facing opposite the press was not guarded. The exposed flywheel was approximately fifty four inches from the floor.
