29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:    (a)  On or about May 22, 2012, for the employees installing roofing materials on a flat roof approximately 15 feet wide and 25 feet long and exposed to a potential fall of approximately 14 feet, located at 1314 West Havens Avenue in Mitchell, South Dakota.    Note:  This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 315002139, Citation 1, Item 1, and was affirmed as a final order on January 25, 2011, with respect to a workplace located at 2201 Sixth Avenue SE in Aberdeen, South Dakota.
