29 CFR 1926.451(c)(2)(iv):  Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    On the south exterior elevation of a building under construction located at 855 21st Street in Vero Beach, Florida:  An employee performing work installing aluminum leaders to existing gutters standing on a scaffold platform supported by a Rough Terrain Fork-lift that was not specifically designed by the manufacturer for such use, on or about May 18, 2012.

29 CFR 1926.451(f)(3):  Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    On the south exterior elevation of a building under construction located at 855 21st Street in Vero Beach, Florida:  A scaffold platform being used by an employee installing aluminum leaders to existing gutters while supported by a Rough Terrain Fork-lift was not inspected by a competent person prior to the work shift for use of platform not recommended or approved by the manufacturer, on or about May 18, 2012.
