29 CFR 1910.110(f)(2)(i):  Containers in storage were not located so as to minimize exposure to excessive temperature rise, physical damage, or tampering by unauthorized persons.    a) Loading Ramp; North Side - Propane containers in storage were not located in an area that would prevent physical damage to the containers and harm to employees.  The storage rack door was bent, and the protective barrier on the door was partially missing.  The containers were exposed to physical damage.

29 CFR 1910.178(a)(4):  Modifications and additions which affected capacity and safe operation were performed by the customer or user without manufacturers written approval, and the capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly.    a) Warehouse - ) Warehouse - The employer did not gain prior approval from the manufacturer, or update label plates prior to having employees operate Power Industrial Vehicles such as, the Bolzoni Auramo double pallet handler attachment SN #08BA004609 which was affixed to the Mitsubishi Forklift, ID #S2983; SN #AF82F31224.     b) Warehouse - The employer did not gain prior approval from the manufacturer, or update label plates prior to having employees operate Power Industrial Vehicles such as, the Bolzoni Auramo double pallet handler attachment SN #08BA004615 which was affixed to the Mitsubishi Forklift, ID #S2779; SN #AF82F10594.    c) Warehouse - The employer did not gain prior approval from the manufacturer, or update label plates prior to having employees operate Power Industrial Vehicles such as, the Push and Pull attachment ID# PP591 which was affixed to the Mitsubishi Forklift, ID# S2378; SN #AF82D01687.
