29 CFR 1926.451(b)(4):     Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm)    a) On or about 05/10/2012 at Holmwood Green and Langworth Hill at the Panther Trace Subdivision employees were observed applying stucco while standing on a platform that rested on the pitched side of the roof approximately 12 feet above the ground.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.      a) On or about 05/10/2012 at Holmwood Green and Langworth Hill at the Panther Trace Subdivision. No safe access to the platform was provided and the employees were observed using the crossbraces as a means of access. This exposed the employees to a fall of approximately 12 feet.

No competent person supervised the erection of the scaffold system used exposing the employees to a fall of approximately 12 feet.

29 CFR 1926.452(n)(3)     Ladders used to support step, platform, and trestle ladder scaffolds shall be placed, fastened, were not equipped with devices to prevent slipping.    a) On or about 05/10/2012 at Holmwood Green and Langworth Hill at the Panther Trace Subdivision employees were observed applying stucco while working from a scaffold system. The trestle scaffold used was free standing, had bent legs, was resting on different wood planks and had no devices to ensure adequate friction to prevent slipping. This exposed the employees to a fall of approximately 12 feet.

29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    a) As observed on or about 05/10/2012 the working level of the trestle scaffold was not fully planked and exposed employees to a fall of approximately 12 feet above the ground at the construction site located at Holmwood Green and Langworth Hill at the Panther Trace Subdivision.    The M&J Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 316306927, citation number 1, item number 1 and was affirmed as a final order on 01/20/2012, with respect to a workplace located at  Riverview , FL 33569.

29 CFR 1926.451(f)(3):     Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    a) On or about 05/10/2012 at Holmwood Green and Langworth Hill at the Panther Trace Subdivision employees were observed applying stucco while working from a scaffold system. The scaffold system had no guardrails, was not fully planked, had unsafe access, the platforms were not secured and less than 18 inches, fitted with a trestle system that was not equipped with slipping control devices .These conditions made the scaffold system unsafe and it used exposing the employees to a fall of approximately 12 feet. These defects could have been identified should the scaffold system was inspected.    The M&J Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(f)(3), which was contained in OSHA inspection number 316306927, citation number 1, item number 1 and was affirmed as a final order on 01/20/2012, with respect to a workplace located at  Riverview , FL 33569.
