29 CFR 1910.1030(c)(1)(v):    The employer, who is required to establish an Exposure Control Plan, solicited input from non-managerial employees responsible for dierct patient care who are potentially exposed to injuries from contaminated sharps in the identification, evaluation and selection of effective engineering and work practice controls and did not document the solicitation in the Exposure Control plan:    (a)  Throughout the facility - The employer did not include employee participation in the determination of safer medical devices, including hypodermic needles, in the company's exposure control plan.

29 CFR 1910.1200(e)(1):     The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    (a)Throughout the facility - Employees were required to work with chemicals, such as but not limited to: GLH Green Line enzymatic liquid for odor elimination, Sporox II sterilizer and disinfectant (containing hydrogen peroxide and phosphoric acid), MetriCide Sterilizer and disinfectant, and PDI Super Sani-Cloth (containing dimethyl ethylbenzyl ammonium chloride).  A written hazard communication program had not been established and/or implemented. The program provided was blank.
