29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). a) Employees were not protected against accidental contact with energized power lines while erecting, dismantling, and working from a ladder jack scaffold within 3 feet of the lines. The power lines consisted of secondary lines having a voltage of 120/240. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(g)(1)(i): Each employee on a ladder jack scaffold was not protected by a personal fall arrest system: a) Employees were not protected from falling from the working level of the ladder jack scaffold. Employees were exposed to falling up to approximately 19.7ft to the ground. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET). Frame Pro, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1) which was contained in OSHA violation number 97436, citation 1, item 1, issued on August 30, 2011 and became final order on or about September 23, 2011 with respect to a workplace located at 2017 Des Plaines Ave., Park Ridge, IL 60068.
