29 CFR 1926.502(b)(1): Top edge height of top rails, or equivalent guardrail system members, were not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: Instances including, but not limited to the following, top rails of guardrails systems were not of appropriate height: a) Jobsite, 1301 Beaver Avenue, Pittsburgh, PA: On or about June 25, 2012, during masonry wall jackhammering activities, the employer did not ensure that the top rail of the guardrail system was of approrpriate height where workers were exposed to fall hazards of approximately 25 feet in that the top rail for the guardrail system was determined to be approximately 34 1/2" in height from the walking working surface.
