29 CFR 1910.101(b): Section 3.3.1, Compressed Gas Association Pamphlet P1 1965, as adopted by 29 CFR 1910.101(b): Compressed gas cylinder(s) ahould be stored in accordance with all local, state and municipal regulations and in accordance with appropriate standards of the Compressed Gas Association and the National Fire Protection Association.     (a)  Bratek Engineering & Manufacturing at 2020 S. Osage St., Denver, CO 80223: On and before 5/21/12, the employer stored oxygen compressed gas cylinders and acetylene cylinders together in the welding shop. Section 3.5.3 of Pamphlet P-1 provides that stored oxygen and fuel gas cylinders should be separated by a minimum of 20 feet, or there should be a fire resistant partition between the oxygen and fuel gas cylinders. 29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      (a)	Bratek Engineering & Manufacturing at 2020 S. Osage St., Denver, CO 80223: On and before 5/21/12, the employer did not protect employees from hazards created by operating an Amada Shearer single press that was unguarded. A portion of the guard was missing. This condition exposed employees to amputation and crush hazards.      (b)	Bratek Engineering & Manufacturing at 2020 S. Osage St., Denver, CO 80223: On and before 5/21/12, the employer did not protect employees from hazards created by operating an unguarded portion of blade of a horizontal band saw. This condition exposed employees to amputation hazards.      Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.95(g)(1):     The employer shall establish and maintain an audiometric testing program as provided in this paragraph by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:    (a) 	Bratek Engineering & Manufacturing at 2020 S. Osage St., Denver, CO 80223: On and before 5/21/12, the employer did not establish and maintain an audiometric tresting program. This condition exposed employees to excessive noise exposure. This condition exposed employees to excessive noise exposure and hearing loss. Osage St., Denver, CO 80223: On and before 5/21/12, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemicals contained within.  Simple Green all-purpose cleaning solution was transferred into smaller containers.  This condition exposed employees to a chemical hazard.
