The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.    (a) Yavapai Apache, Camp Verde, AZ, First-Man Street, Unit 105:  On or thereabout April 26, 2012, employer did not      provide an effective training program ensuring an employee recognized and minimize the hazard of using of six foot frame step ladder.  The ladder was missing information such as, but not limited to ladder's model number, type, grade, duty rating, size and highest standing level.
