On or about 5/16/2012, an employee without means of fall protection was exposed to a fall hazard of up to 16 feet while accessing a roof and a scaffold to install architectural foam at a construction site located at 350 South State Road 7, Margate FL. 33068. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about 5/16/2012, an employee who was exposed to a fall hazard of up to 16 feet while accessing a roof to install architectural foam without means of fall protection was not provided with training to recognize fall hazards and procedures to follow to minimize these hazards at a construction site located at 350 South State Road 7, Margate FL. 33068.
