29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: On or about 05-16-12, at 909 Flagger Ave. Key West, FL 33040, a scaffold that was not fully planked was being used by employees to hang rafters, exposing employees to the hazard of falling 10 feet four inches to the ground.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: On or about 05-16-12, at 909 Flagger Ave. Key West, FL 33040, employees working on a scaffold that was not fully planked had not been trained to recognize fall hazards and were exposed to a fall hazard of 10 feet 4 inches to the ground.
