29 CFR 1926.403(b)(2):     Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:    Employees using a metal receptacle box were exposed to electrical hazards.  The employees were plugging extension cords into an unmounted four receptacle handy box and the metal box was not used according to its listing requirements.

29 CFR 1926.501(b)(13):      Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provided an alternative fall protection measure:    Employees replacing a residential roof were exposed to fall hazards of approximately 24 feet.  No fall protection was utilized.

29 CFR 1926.503(a)(1):      The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Employees replacing a residential roof were exposed to fall hazards of approximately 19 feet.  A training program was not provided to enable each employee to recognize the hazards of falling.

29 CFR 1926.1053(b)(1):      Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    Employees using a portable extension ladder to access a roof were exposed to fall hazards.  The step ladder side rails extended approximately one foot above the upper landing surface.

The employees had not been trained so far as to enable each employee to recognize hazards related to ladders and procedures to follow in order to minimize those hazards.
