29 CFR 1910.1200(g)(1): Chemical manufacturers and importers shall obtain or develop a material safety data sheet for each hazardous chemical they produce or import. Employers did not have a material safety data sheet in the workplace for each hazardous chemical which they use.    At the jobsite; the employer did not maintain a material safety data sheet on the jobsite for Holcim Type S masonry cement and Holcim Type I Portland cement that contained hazardous substances such as crystalline silica and hexavalent chromium, exposing employees to inhalation hazards.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold were not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).    West side of the structure; employees performed masonry work from a scaffold that did not have top or mid rails nor end rails, exposing employees to fall hazards at approximately 13.5 feet.

29 CFR 1926.452(c)(2): Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections shall be secured.    West side of the structure; employees performed masonry work from a scaffold that did not have cross bracing, exposing employees to fall hazards at approximately 13.5 feet
