29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about May 22, 2012, an employee who was working on the ground level while other employees were working on the roof, was not wearing a hard hat. This employee was exposed to the hazard of being struck by falling materials and/or tools.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b): On or about May 22, 2012, on the south (front) side of the house, employees were on the roof of the residence installing a new roof and were not using any type of fall protection. The employees were exposed to fall hazards of approximately 8 feet to the ground below.
