29 CFR 1928.57(a)(6)(iii): The employer did not ensure that employees stopped engine, disconnected the power source, and waited for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment, except where the machine must be running to be properly serviced or maintained, in which case the employer instructed employees as to all steps and procedures which are necessary to safely service or maintain the equipment; On or about May 20, 2012 at 15 Black, LLC. the employer did not ensure that the main power switch for the sprinkler system was in the off position prior to servicing. Employees were exposed to potential struck-by and electrical hazards.

On or about May 20, 2012 at 15 Black, LLC. the employer did not provide training in the safe operation and servicing of equipment with which employees will be involved, including but not limited to sprikler electrical system. Employees were exposed to potential struck-by and electrical hazards.
