29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.    At jobsite located at 10509 S. 21st St, Wichita, KS, employee(s) were exposed to struck by hazards of the spoil pile, in that the spoil pile was not kept 2 feet from the edge of the excavation, nor were retaining devices of any type used to protect employee(s).

At jobsite located at 10509 S. 21st St, Wichita, KS, employee(s) were exposed to trench cave-in, in that the competent person had allowed employee(s) to enter a trench without meeting all the elements of sloping/benching (OPTION (2), -3 OR (4)) for a Type B soil.
