29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone: (a) Tiley Roofing, Inc., @ 1112 N. Speer Blvd., Denver, CO 80204: On and before 5/24/12, employees engaged in roofing activities on a flat, commercial roof which measured less than 50ft in its least dimension were not protected from a fall hazard. This condition exposed employees to a fall hazard of approximately 15.4ft as measured from ground to eve.

29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which took the monitors attention from the monitoring function: (a) Tiley Roofing, Inc., @ 1112 N. Speer Blvd., Denver, CO 80204: On and before 5/24/12, the safety monitor assisted in roofing activities while outside the warning line system on a flat, commercial roof. Additionally, the safety monitor failed to monitor employees while they conducted roofing activities outside the warning line system. This condition exposed employees to a fall hazard of approximately 15.4ft as measured from ground to eve.
