29 CFR 1926.102(a)(2): Eye and face protection equipment required by this Part shall meet the requirements specified in American National Standards Institute, Z87.1-1968, Practice for Occupational and Educational Eye and Face Protection. a. On the South side of the building, the employee was chipping and grinding stone without ANSI Z87.1-1968 approved eye protection, thereby being exposed to a struck-by hazard.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). a. On the South side of the building, employees were working in an aerial lift at 23' without having the fall protection lanyard connected, thereby being exposed to a fall hazard.
