29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: (a) Cold side of main shop: on 5/22/12 and at times prior thereto, a ladder leading to a mezzanine approximately 8' 10" from the next lower level was not equipped with a system to prevent workers from walking into the opening. NOTE: Abatement certification IS required for this item.

29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: (a) Cold side of main shop: on 5/22/12 and at times prior thereto, the mezzanine edge was guarded only by a loose chain with deep sags between supports. (b) Warm side of main shop: on 5/22/12 and at times prior thereto, the mezzanine edge did not have a standard railing along edges measuring 98.25" and 125". NOTE: Abatement certification IS required for this item.
