29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) On or about 5/24/12 at a facade improvement project at Marco's Pizza located at 289 Main Street, Arcade, New York; employees were using a 7 1/4" Skil-saw to cut building materials. The saw had been repaired by splicing a section of extension cord to the original cord for the saw. The repair was not in accordance with the manufacturer's requirements. NO ABATEMENT CERTIFICATION REQUIRED

The were 2 employees on a scaffold platform that was rated for only one employee. NO ABATEMENT CERTIFICATION REQUIRED
