29 CFR 1910.147(d)(4)(i):     Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    a. Some molding machine cleaning was done without a lock applied by the employee cleaning.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.147(d)(6):     Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished:    a. When working on molding machines, testing for energy isolation after lockout was not done according to procedures.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. 29 CFR 1910.147(c)(4)(ii):     Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h):    a. A specific lockout procedure was not written for at least one molding machine (#103).    b. Lockout procedures were not specific for the testing of energy isolation; the specific device, method, control button, etc. was not specified such as on the 600 Ton Cincinnati molding machine, mod: VH-600-54.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).
