29 CFR 1926.100(a):     Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    (a)  At the 2803 E. Oklahoma Avenue, Milwaukee, Wisconsin jobsite, the employer did not ensure employees performing ground clean-up utilized head protection. Employees were exposed to struck-by hazards.

(a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY)        (a)  At the 2803 E. Oklahoma Avenue, Milwaukee, Wisconsin jobsite, employee(s) using the Bostitch nail gun did not utilize eye protection, exposing them to struck-by hazards.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)      (a) On or about 05/25/2012 at the 2803 E. Oklahoma Avenue, Milwaukee, Wisconsin jobsite, employee(s) were engaged in residential roofing activities on an approximate 8:12 pitch roof without the use of conventional fall protection, exposing employees to maximum 12.8 foot fall hazards.     MILWAUKEE HOME IMPROVEMENTS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD [29 CFR 1926.501(b)(13)] WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311400477, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 8, 2008, WITH RESPECT TO A WORKPLACE LOCATED AT 4620 N MORRIS BLVD, WHITEFISH BAY, WISCONSIN.

Oklahoma, Milwaukee, Wisconsin jobsite, the employer did not provide employee(s) engaged in residential roofing activities training on identifying and minimizing fall hazards. Employees were exposed to a maximum 12.8 foot falls.
