OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    a) 234 Gas Plant Rd., Grand Isle, LA; On or before May 9, 2012, the employer failed to ensure that a V-35 Hydraulic Vibratory Pile Driver was properly secured from falling during pile driving operations. The approximately 4-ton pile driver was left standing upright disconnected from the crane while the crane retrieved another pile. Employee(s) working in the vicinity were exposed to the hazard of being struck-by the pile driver in the event it fell over.
