29 CFR 1926.403(b)(1):     Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  On or about May 22, 2012, at 5810 Swift Creek Court, Suwanee, Georgia:    Employees framing a house were exposed to electric shock hazard while using a damaged extension cord that was cut and had the insulation pulled away from the plug to power a Dewalt 15 Amp circular saw.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.    On or about May 22, 2012, at 5810 Swift Creek Court, Suwanee, Georgia:    a) Employees framing a house were exposed to a fall hazard of greater than 25-feet when the employees working on the roof trusses did not use a means of fall protection.     b) Employees framing a house were exposed to fall hazards of approximately 10.2 feet and greater when working within six feet of unguarded window openings on the second floor.     c) Employees framing a house were exposed to a fall hazard of approximately 18.5-feet from an unguarded floor opening.
