29 CFR 1926.1053(b)(8): Ladders placed in any location where they can be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, shall be secured to prevent accidental displacement, or a barricade shall be used to keep the activities or traffic away from the ladder. (a) On May 25, 2012, the ladder being used by the two employees for access and egress from the 10/12 pitch roof was not secured to prevent accidental displacement nor was it barricaded to prevent driveway traffic from exposing employees to a 19' fall hazard.

29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. (a) On May 25, 2012, site inspections were not conducted by a competent person designated by the employer in that employees were engaged in reroofing activities on a residential structure located at 7345 Elbrook Road were not utilizing any type of fall protection nor were the employees securing the ladder being utilizing on site. Employees were exposed to a 19' fall hazard. Espinoza Construction was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA inspection number 315337675, citation number 1, item number 1, issued on February 28, 2011.
