29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)      At the site, a tubular welded frame scaffold being used by employees to install new siding, had;    A) The second level, measuring approximately equal to or greater than 10' above  the ground level, only utilized one fabricated scaffold plank,    B) The first level, measuring approximately equal to or greater than 5' above the ground level, only utilized 2 6" solid sawn lumber planks,    leaving approximately 35" between the decking edge and the front of the uprights.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.        At the site, employees used the tubular welded frame scaffold uprights to gain access to the tiers and also to exit the scaffold.

29 CFR 1926.451(f)(3):     Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:      At the site, the scaffold was equipped with casters to allow horizontal movement of the scaffold. However, the caster's latches were broken, thus preventing the locking of the casters to prevent movement while employees were working from the tiers of the scaffold. As stated by the employer, no qualified person was onsite to inspect the scaffold for defects or damage.

29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.      At the site, an employee was exposed to fall hazards approximately equal to or greater than 10' from a tubular welded frame scaffold.
