29 CFR 1926.21(b)(2):     The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:    (a) Jobsite, Route 4, East Greenbush, on or about May 25, 2012 - employee(s) were not instructed in the recognition and avoidance of jobsite hazards including but not limited to: location of Ingersol Rand-Light Source temporary lighting units with respect to proximity of overhead high voltage power lines.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:      a) Jobsite, Route 4, East Greenbush, on or about May 25, 2012 - Ingersol Rand model Light Source temporary lighting units were placed directly below high voltage power lines, such that the mast of the lighting unit could contact the energized line during operation.
